Enrollment School Forms Student Uniform Saint Mary Star of the Sea School does not discriminate and accepts applications from students of all race and ethnic origin who possess the motivation, ability and character that enable them to succeed. The list of categories below outline the forms necessary to enroll your child based on your registration needs.  Print the forms at home, complete all required sections, and submit to us via U.S. mail, fax them to us, or leave them with our staff at the school. In order for a child to be considered, all forms must be complete and both a $50 registration fee and $100 book fee included.
